Background:
The Impella CP device provides vital mechanical circulatory support in cardiogenic shock but carries a risk of acute limb ischemia, particularly in patients with peripheral artery disease.
Case Summary:
A 70-year-old woman with acute ST-segment elevation myocardial infarction experienced cardiogenic shock, requiring Impella CP support. After implantation, she was at high risk for acute limb ischemia. To maintain distal perfusion, a temporary extracorporeal bypass system was created using femoral artery sheaths and connecting tubing. She remained in stable condition without ischemic complications. The Impella CP and bypass system were successfully removed after hemodynamic recovery.
Discussion:
Acute limb ischemia is a serious complication of Impella use. This case highlights that a temporary bypass system is an effective intervention to prevent limb loss in high-risk patients.
Take-Home Messages:
Timely recognition and intervention can prevent limb ischemia in Impella-supported patients. Temporary bypass circuits provide a practical solution to maintain distal perfusion.
