    Summary

Povidone-iodine antiseptic caused a patient to develop a rash. Patch testing confirmed the allergy to povidone-iodine, but not to elemental iodine.

    Area of Science:

    • Dermatology
    • Allergology
    • Clinical Medicine

    Background:

    • Povidone-iodine is a widely used antiseptic in surgical settings.
    • Adverse skin reactions to antiseptics can occur, necessitating accurate diagnosis.

    Observation:

    • A patient presented with an erythematous, papular, and vesicular rash following the application of 10% povidone-iodine solution.
    • The rash appeared after the surgical antiseptic was used.

    Findings:

    • Patch testing demonstrated a positive reaction to both 10% and 5% povidone-iodine solutions in water.
    • Control patch tests with 25 different substances were negative.
    • Patch testing with 0.5% elemental iodine in ethanol yielded negative results, suggesting the allergy is to the povidone-iodine complex rather than free iodine.

    Implications:

    • This case highlights povidone-iodine as a potential contact allergen.
    • Distinguishing allergy to the povidone-iodine complex from elemental iodine is crucial for patient management.
    • Further investigation into the specific components of povidone-iodine responsible for allergic reactions may be warranted.
