Summary

Fresh embryo transfers on day 5 significantly increase pregnancy success rates compared to day 4 transfers. Day 5 transfers are recommended, particularly for women over 40, to optimize in vitro fertilization outcomes.

Background:

  • Most in vitro fertilization (IVF) studies compare cleavage-stage to blastocyst-stage embryos.
  • Limited research exists on comparing pregnancy rates between day 4 and day 5 fresh embryo transfers within antagonist-regulated IVF cycles.

Purpose of the Study:

  • To compare pregnancy rates between day 4 and day 5 fresh embryo transfers.
  • To identify factors influencing pregnancy success in fresh, antagonist-regulated IVF cycles.

Main Methods:

  • Retrospective cohort study of 3681 fresh embryo transfer cycles (2019-2021).
  • Patients divided into day 4 (Group 1) and day 5 (Group 2) transfer groups.
  • Subgroup analyses based on patient age and oocyte retrieval numbers; GnRH antagonist protocol used.

Main Results:

  • Day 5 transfers yielded higher pregnancy rates (65.3%) compared to day 4 transfers (54.2%).
  • Single embryo transfer on day 5 increased pregnancy success by 1.8 times; double embryo transfer by 1.6 times.
  • Day 5 transfers showed a 1.9-fold increase in pregnancy success regardless of age, oocyte count, or embryo number.

Conclusions:

  • Fresh embryo transfers on day 5 are superior to day 4 transfers for improving pregnancy rates.
  • Day 5 transfers should be prioritized, especially for women over 40, to enhance IVF success.
