Purpose of the Study:

  • To investigate the clinical progression and long-term outcomes of patients diagnosed with biopsy-proven PCNSV.

Main Methods:

  • Retrospective review of clinical data from eight patients with biopsy-confirmed PCNSV.
  • Patients underwent thorough evaluation to exclude secondary causes of vasculitis.

Main Results:

  • Presenting symptoms included focal neurological deficits, seizures, headache, and cognitive decline.
  • MRI revealed multifocal signal changes, microhemorrhages, and contrast enhancement.
  • Immunosuppressive therapy, including methylprednisolone and cyclophosphamide, led to disease control, but relapses occurred, necessitating treatment escalation. Rituximab showed benefit in select cases.

Conclusions:

  • PCNSV presents variably and poses diagnostic challenges.
  • Effective immunosuppressive therapy is vital but carries risks.
  • Histopathological confirmation, early aggressive treatment, and close clinico-radiological monitoring are key for successful long-term management and remission.
