Background:

  • Objective performance indicators (OPIs) in robotic surgery correlate with surgeon experience and patient outcomes.
  • Evaluation of OPIs in bariatric surgery, specifically robotic Roux-en-Y gastric bypass (r-RYGB), is limited.

Purpose of the Study:

  • To determine if OPIs can effectively differentiate experienced surgeons from trainees during r-RYGB.
  • To explore the utility of OPIs in assessing surgical skill in bariatric procedures.

  • Kinematic and event data were collected from 31 r-RYGB procedures (2 attending surgeons, 6 trainees) using the da Vinci Surgical System.
  • Data included console active time, instrument movement speed, bimanual dexterity, and workspace volume rate.
  • Videos were annotated across eight surgical steps for OPI generation and analysis.

Main Results:

  • Significant OPI differences between experienced and trainee surgeons were observed in 3 of 8 procedural steps.
  • Experienced surgeons showed greater bimanual dexterity and varied instrument speed during dissection and gastric pouch creation.
  • Trainees exhibited lower console active time during specific steps like gastric pouch creation and hand-sewn anastomosis.

Conclusions:

  • Step-specific OPIs effectively distinguish experienced surgeons from trainees in robotic surgery.
  • Further validation of OPIs could enable objective assessment of trainee performance.
  • OPIs may inform surgical training progression and decisions regarding surgeon autonomy.
