Summary
This study found that cyclophosphamide-based regimens, with or without bortezomib, yield more stem cells for multiple myeloma patients. These regimens are effective for stem cell mobilization and collection.
Area of Science:
- Hematology
- Oncology
- Stem Cell Transplantation
Background:
- Stem-cell mobilization in multiple myeloma typically uses G-CSF with or without cyclophosphamide (Cy) chemotherapy or Plerixafor.
- Pre-clinical studies suggest proteasome inhibitors can aid mobilization.
- Previous findings indicated Bortezomib (Bort) plus Cy improved stem-cell yield.
Purpose of the Study:
- To compare the efficacy of four different stem-cell mobilization regimens in multiple myeloma patients.
- To determine the proportion of patients achieving a CD34+ dose ≥ 5 × 10^6/kg in the first apheresis.
- To assess median CD34+ yield, total stem-cell yield, and mobilization failure rates across regimens.
Main Methods:
- Retrospective analysis of 200 multiple myeloma patients (aged 18-65) undergoing stem-cell mobilization from September 2007 to December 2022.
- Four mobilization regimens were evaluated: Bortezomib + G-CSF (Group 1), G-CSF + Plerixafor (Group 2), Bortezomib-Cyclophosphamide-G-CSF (Group 3), and Cyclophosphamide + G-CSF (Group 4).
- Intention-to-treat analysis focused on CD34+ cell yield and mobilization failure.
Main Results:
- The proportion of patients achieving ≥ 5 × 10^6 CD34+ cells/kg in the first apheresis was 26%, 53%, 69%, and 63% for Groups 1-4, respectively (p=0.0001).
- Median CD34+ yield in the first harvest was 3.62, 5.20, 6.04, and 6.05 × 10^6/kg (p=0.00004).
- Median total stem-cell yield was 5.73, 6.17, 9.14, and 8.23 × 10^6/kg (p<0.00001), with Group 3 showing the highest yield. Mobilization failure rates were 7%, 3%, 0%, and 2% (p=NS).
Conclusions:
- Cyclophosphamide-based chemotherapy regimens, with or without Bortezomib, offer superior total stem-cell yield compared to G-CSF + Plerixafor.
- These regimens are comparable to G-CSF + Plerixafor for single-apheresis yield.
- Adding Bortezomib to cyclophosphamide-based regimens may further enhance stem cell collection in multiple myeloma.

Stem Cell Therapy for Tissue Regeneration
4.2K
Stem cell therapy is a method used in regenerative medicine to repair and restore function to damaged tissues and organs. Stem cells have the potential to proliferate and differentiate into various tissue types, making them ideal candidates for tissue regeneration. For example, hematopoietic stem cell transplants are commonly used in blood cancer treatment to replenish damaged bone marrow and restore healthy blood cells.

Differentiation of Common Myeloid Progenitor Cells
3.3K
Common myeloid progenitors (CMPs) are oligopotent cells that can differentiate into granulocytes and macrophages. Granulocytes and macrophages are essential for protecting the body against bacterial, viral, or fungal infections. They migrate from the bone marrow into the circulating blood to reach specific tissue sites where they differentiate and help in immune surveillance. However, they survive only for a few days and must be continuously made available to the organism to maintain a robust...
