A new index, the Atlantic Pouchitis Index (API), was developed and validated to accurately measure pouchitis disease activity. This reliable and responsive tool combines endoscopic and histologic findings for better therapy evaluation in pouchitis patients.
Area of Science:
- Gastroenterology
- Inflammatory Bowel Disease Research
- Clinical Trial Methodology
Background:
- Existing indices for measuring pouchitis activity lack full validation.
- There is a need for a reliable and responsive tool to assess pouchitis disease activity.
Purpose of the Study:
- To develop and externally validate a novel index for measuring pouchitis disease activity.
- To integrate endoscopic and histologic findings into a single, comprehensive index.
Main Methods:
- A novel index, the Atlantic Pouchitis Index (API), was developed using linear regression.
- The API was assessed for reliability (intraclass correlation coefficient) and responsiveness (area under the receiver operating characteristic curve) using data from 98 patients.
- External validation was performed using data from the EARNEST vedolizumab trial.
Main Results:
- The API combines the Simple Endoscopic Score for Crohn's Disease and the Robarts Histopathology Index.
- The API demonstrated excellent intra-rater (ICC, 0.88) and substantial inter-rater reliability (ICC, 0.72).
- The API showed high responsiveness (AUROC, 0.95) and outperformed existing endoscopic indices.
Conclusions:
- The development and validation of the Atlantic Pouchitis Index (API) represent a significant advancement in pouchitis disease assessment.
- The API provides an objective, reliable, and responsive instrument for evaluating therapy effectiveness.
- This index can be utilized in both research and clinical settings for improved patient management.

It requires a multifaceted approach, starting with a detailed physical examination to pinpoint the location and nature of the pain and identify any associated symptoms. Laboratory tests play a crucial role. A complete Blood Count (CBC) typically reveals leukocytosis (an increased number of...
