Background:

Technological advancements are transforming healthcare delivery.
There is a growing trend towards home-based care models.
Integrating technology into home care offers potential for improved patient outcomes.

Purpose of the Study:

To synthesize research on emerging home-based care models and associated technologies.
To identify research, policy, and practice priorities in this evolving field.

Main Methods:

A scoping review was conducted.
Methodologies included Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews (PRISMA-ScR).
The Joanna Briggs Institute methodology and Arksey and O'Malley framework were also employed.

Main Results:

105 studies were included from 3,154 screened.
Common technologies: wearable devices (31%), mobile apps (28%), remote patient monitoring (26%).
Prevalent care models: person-centered self-care (56%), continuum-of-care (50%).

Conclusions:

Nursing plays a critical role in the integration of emerging technologies in home-based care.
This includes contributions to policy development, research initiatives, and practical implementation.
Future priorities should focus on leveraging technology to optimize home-based care delivery.
