Research Questions:
Is the 3.0 Tesla magnetic resonance imaging (MRI) high-resolution tissue spectroscopy (HISTO) sequence efficacious as a non-invasive method for assessing metabolic-associated fatty liver disease (MAFLD) in patients with polycystic ovary syndrome (PCOS)? Are there associations between the HISTO-proton density fat fraction (PDFF) value and liver fat infiltration, liver fat content, clinical indicators and body composition?
Design:
MRI HISTO sequences of the liver were acquired in 28 patients with PCOS and 37 age-matched healthy controls. Fasting blood chemistry including liver function tests, lipid profile, glucose and insulin, morphometric characteristics, and body composition analysis were recorded. Spearman correlation and multivariate linear regression analysis were performed to determine associations between clinical data and HISTO-PDFF values.
Results:
Hepatic HISTO-PDFF values were significantly higher for patients with PCOS compared with healthy controls (P < 0.001), and this was also found in a subgroup analysis that accounted for body weight (P = 0.007). Multivariate linear regression analysis suggested that alanine aminotransferase and fasting insulin concentrations were independent predictors of the hepatic HISTO-PDFF value.
Conclusion:
The MRI HISTO sequence is a non-invasive and sensitive method to assess hepatic steatosis in women with PCOS, facilitating early clinical intervention.
