Clinical Features Associated With Fatigue in People With Fibrotic Interstitial Lung Disease: Cross-Sectional Study

Fatigue in fibrosing interstitial lung diseases (fILD) is linked to breathlessness and reduced physical activity, not oxygen levels. Understanding these factors can improve quality of life for fILD patients.

- 116 participants with stable fILD and exertional desaturation were included.
- Fatigue Severity Scale (FSS) assessed fatigue; independent variables included age, BMI, FVC%, Dyspnoea-12 score, daily steps, nadir SpO2, corticosteroid use, and obstructive sleep apnea.
- Univariate and multivariate regression analyses were performed to identify associations.
Main Results:
- 62% of participants reported substantial fatigue (FSS ≥ 36).
- No significant relationship was found between fatigue (FSS) and nadir SpO2 during the 6-minute walk test or daily life.
- Higher fatigue scores correlated with increased breathlessness (Dyspnoea-12) and decreased daily physical activity (steps).
Conclusions:
- Fatigue in fILD is primarily associated with breathlessness and reduced physical activity.
- Exertional oxyhaemoglobin desaturation does not appear to be a significant contributor to fatigue in this patient group.
- Findings suggest focusing on managing dyspnea and promoting physical activity to alleviate fatigue in fILD.
