Surgeon use of patient-reported outcome measures (PROMs) improved substantial clinical benefits (SCB) in knee replacement patients. However, PROM utilization did not significantly impact outcomes for hip replacement patients.

Background:

Purpose of the Study:

  • To evaluate the impact of surgeon utilization of PROMs on achieving minimum clinically important differences (MCID), substantial clinical benefits (SCB), and patient-reported global impression of change (PGIC).
  • To compare outcomes between primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) patients based on surgeon PROM usage.

Main Methods:

  • Analysis of 7,441 primary TJA patients (4,458 THAs, 2,983 TKAs) with pre- and post-operative PROMs.
  • Surgeons classified as PROM users or nonusers based on referencing pre-operative PROMs.

  • No significant differences in MCID, SCB, or PGIC were found between PROM user and nonuser groups for total hip arthroplasty (THA).

Conclusions:

  • Surgeon reference to PROMs in TKA is associated with improved SCB, indicating potential benefits of PROM integration.
  • The lack of significant impact in THA may be due to generally higher and more predictable patient satisfaction in this group.
  • PROM utilization may enhance outcomes in specific TJA populations, warranting further investigation.
