We report the first UK case of a robot-assisted free flap reconstruction using the Symani Surgical System for microvascular anastomosis. Following resection of a pre-sternal DFSP, a DIEP flap was used for chet wall reconstruction, with the arterial anastomosis performed robotically. The procedure was completed safely, marking a significant step in integrating robotic technology into open microsurgery. Despite added setup time, the system offered enhanced precision and ergonomics, supporting its role in future complex reconstructive and supermicrosurgical procedures.
