Abstract:
Polycystic Ovarian Syndrome (PCOS) is the most prevalent hormonal disorder that affects metabolism, ovulation and menstrual cycles. SIRT1 belongs to the Sirtuin family, a group of NAD+ dependent class 3 histone deacetylases. This review explores the correlation between SIRT1 and PCOS and associated mechanisms of action like Insulin resistance, oxidative stress, inflammation and autophagy. Extensive studies have been done on SIRT1, how it was downregulated and contributed to conditions like insulin resistance. How activation of SIRT1 helps by deacetylating the different responsible transcriptional factors is discussed in the mechanisms briefly. Additionally, we discussed the molecular structures of the existing natural and synthetic SIRT1 activators and their roles, which were previously reported, from the chemistry perspective. This review gives insights into the challenges and advantages of SIRT1, which can be a beneficial therapeutic target for PCOS.
Insights
Polycystic Ovarian Syndrome (PCOS) involves SIRT1 dysregulation, impacting insulin resistance and inflammation. Activating SIRT1 offers a potential therapeutic strategy for PCOS management.

Background:
- Polycystic Ovarian Syndrome (PCOS) is a common endocrine disorder affecting reproductive and metabolic functions.
- Sirtuin 1 (SIRT1), an NAD+-dependent deacetylase, plays a role in cellular metabolism and stress response.
- Dysregulation of SIRT1 has been implicated in the pathophysiology of PCOS.
Purpose of the Study:
- To review the correlation between SIRT1 and PCOS.
- To elucidate the mechanisms linking SIRT1 to PCOS, including insulin resistance, oxidative stress, inflammation, and autophagy.
- To discuss the chemical structures and roles of SIRT1 activators as potential therapeutic agents for PCOS.
Main Methods:
- Literature review of studies on SIRT1 and PCOS.
- Analysis of molecular mechanisms involving SIRT1 in PCOS.
- Review of existing natural and synthetic SIRT1 activators and their chemical properties.
Main Results:
- SIRT1 is frequently downregulated in PCOS, contributing to insulin resistance and other metabolic disturbances.
- SIRT1 activation, through deacetylation of key transcription factors, can ameliorate PCOS-related pathologies.
- Various natural and synthetic compounds exhibit SIRT1-activating properties with potential therapeutic applications.
Conclusions:
- SIRT1 is a critical molecular target for PCOS intervention.
- Targeting SIRT1 activation presents a promising therapeutic avenue for managing PCOS and its associated metabolic complications.
- Further research into SIRT1 activators could lead to novel PCOS treatments.

Hormonal Control of the Ovarian Cycle
Before puberty, the hypothalamus releases GnRH in a low frequency, low amplitude pulsatile manner. This along with the immature hypothalamic-pituitary-gonadal axis activity, results in low estrogen levels and the absence of a fully functional ovarian cycle. At puberty, GnRH secretion increases in both frequency and...
