Summary

Primary iliac bone tuberculosis (TB) is rare and difficult to diagnose. GeneXpert testing rapidly confirmed TB in a patient with multiple comorbidities, enabling timely treatment.

Background:

  • Primary iliac bone TB is an uncommon manifestation, often posing diagnostic challenges due to its mimicry of other bone pathologies.

Observation:

  • A 63-year-old patient on peritoneal dialysis with hypertension and type II diabetes (complicated by retinopathy and kidney disease) presented with symptoms suggestive of iliac bone infection.
  • Magnetic resonance imaging (MRI) indicated osteomyelitis in the iliac bone.

Findings:

  • Real-time polymerase chain reaction (PCR) using the GeneXpert system on a gluteal collection sample definitively diagnosed tuberculosis.
  • The patient's comorbidities included hypertension, type II diabetes, diabetic retinopathy, and diabetic kidney disease.

Implications:

  • Advanced molecular diagnostics like GeneXpert offer rapid and accurate identification of TB, crucial for rare presentations.
  • Early diagnosis facilitates prompt treatment initiation, potentially improving outcomes in complex patient cases.
