Summary

Three-dimensional MR elastography (3D-MRE) shows promise for non-invasive intraocular pressure (IOP) assessment. Anterior segment shear stiffness (SS-AS) measured by 3D-MRE is a reliable biomarker for predicting IOP, independent of ocular geometry.

Background:

  • Elevated intraocular pressure (IOP) is a primary risk factor for glaucoma.
  • Accurate IOP assessment is crucial for glaucoma diagnosis and management.
  • Current methods like Goldmann applanation tonometry (GAT) require corneal contact, while non-contact tonometry (NCT) is influenced by ocular geometry.

Purpose of the Study:

  • To evaluate three-dimensional MR elastography (3D-MRE) as a novel, non-invasive method for assessing intraocular pressure (IOP).

Main Methods:

  • A prospective study involving 29 healthy volunteers.
  • Phase-contrast spin-echo echo planar MRE at 3.0 T was used to measure shear stiffness (SS) at 90 Hz.
  • Ocular geometry parameters were measured using optical biometry.
  • Statistical analyses included Wilcoxon signed rank test, multivariable regression, and intraclass correlation coefficient (ICC).

Main Results:

  • Shear stiffness of the anterior segment at 90 Hz (SS-AS@90 Hz) demonstrated excellent repeatability and inter-/intra-observer agreement (ICC ≥ 0.889).
  • SS-AS@90 Hz showed a stronger correlation with Goldmann applanation tonometry (IOPGAT) (r=0.64) compared to non-contact tonometry (IOPNCT) (r=0.59).
  • Multivariable regression identified SS-AS@90 Hz (β=0.53) and IOPNCT (β=0.40) as independent predictors of IOPGAT, explaining 58.3% of the variance. SS-AS@90 Hz was independent of most ocular geometric parameters.

Conclusions:

  • SS-AS@90 Hz measured by 3D-MRE is a robust biomarker for predicting intraocular pressure (IOP).
  • This non-invasive method shows potential for glaucoma assessment, independent of common ocular geometric factors.
