Background:

  • Acute bilateral retinal and choroidal detachment is rare.
  • Uveal effusion syndrome (UES) and central serous chorioretinopathy (CSC) can present with serous retinal detachment.

Purpose of the Study:

  • To describe an atypical presentation of acute bilateral retinal and choroidal detachment.
  • To discuss the diagnostic and therapeutic challenges in such cases.

Main Methods:

  • Case report of a 57-year-old male with acute bilateral retinal detachment.
  • Multimodal imaging including fundoscopy, echography, fluorescein angiography (FA), and optical coherence tomography (OCT).
  • Treatment involved initial steroid boluses followed by laser photocoagulation.

Main Results:

  • Bilateral annular retinal and choroidal detachment was observed.
  • Initial steroid treatment showed minimal improvement.
  • Laser photocoagulation of hyperfluorescent spots identified on FA led to gradual anatomical and functional recovery.
  • Complete resolution of detachment and dramatic vision improvement over seven months.

Conclusions:

  • This case represents an atypical presentation of type-3 uveal effusion syndrome with CSC-like features.
  • Multifocal laser photocoagulation may be a viable treatment option for such presentations.
  • The case highlights the potential for dramatic recovery even in atypical scenarios.
