Summary

The sting challenge test (SCT) accurately predicts venom immunotherapy (VIT) effectiveness, showing a high negative predictive value. This test is safe and useful for assessing if VIT has worked for patients with Hymenoptera allergies.

Background:

The sting challenge test (SCT) is the gold standard for evaluating venom immunotherapy (VIT) efficacy.
However, its precise predictive value in real-world clinical settings remains under investigation.

Purpose of the Study:

To evaluate the predictive value of the SCT in patients undergoing VIT.
To assess the safety and utility of SCT for determining VIT effectiveness.

Main Methods:

A multicenter retrospective observational study included 261 patients receiving VIT.
Data collected included patient demographics, diagnosis, immunotherapy details, SCT outcomes, and subsequent field sting events.
SCTs were performed using various Hymenoptera venoms, with results and field sting data analyzed.

Main Results:

The study analyzed 372 SCTs and 306 subsequent field stings across 261 patients.
The SCT demonstrated a high negative predictive value (NPV) of 94.9% for predicting negative field stings.
The positive predictive value (PPV) for the SCT was 33.3%, indicating limitations in predicting positive field stings.

Conclusions:

The sting challenge test (SCT) is a safe procedure for patients undergoing venom immunotherapy.
The high NPV of the SCT strongly supports its utility in assessing the effectiveness of VIT.
The SCT is a valuable tool for clinicians managing patients with Hymenoptera venom allergies.
