Management of Ulna Nonunion with Radial Head Dislocation in a Formerly Treated Pediatric Monteggia Fracture: A Case

Case:
The management of a chronic Monteggia with ulna nonunion is complex and sparsely reported. We present a case of a 10-year-old child with a chronic Monteggia fracture and ulna nonunion following elastic nailing, with persistent radial head dislocation. The condition was treated with distraction angulation using an iliac crest autograft, fixed with a prebent dynamic compression plate, and an open reduction of the radial head. The patient achieved excellent functional results at the two-year follow-up.
Conclusion:
This case underscores the challenges of using elastic nailing for Monteggia fractures and the effectiveness of distraction angulation with autograft in managing ulna nonunion.
