Background:

Keratoconus, a progressive corneal ectasia, can cause severe visual impairment.
Cataracts can further compromise vision in keratoconus patients, impacting mental health.
This case involves an 80-year-old woman with severe bilateral keratoconus and primary angle closure glaucoma.

Observation:

The patient presented with blurred vision and elevated intraocular pressure.
Preoperative assessment revealed high astigmatism due to keratoconus (-10.00DC OD, -9.00DC OS).
She underwent bilateral cataract surgery with monofocal toric intraocular lens implantation.

Findings:

Postoperatively, astigmatism was significantly reduced (-2.50DC OD, -1.50DC OS).
Vision improved markedly despite coexisting glaucoma and tunnel vision.
The patient reported increased independence and improved quality of life.

Implications:

Toric intraocular lens implantation can effectively correct significant astigmatism in selected keratoconus patients.
Meticulous surgical planning, biometry, and patient counseling are crucial for successful outcomes.
This approach offers a viable option for enhancing vision and well-being in patients with combined keratoconus and cataracts.
