The Other Side of Tamponade: RP Impella Support in Pericardial Decompression Syndrome

Background:
Pericardial decompression syndrome (PDS) is a rare complication of pericardiocentesis, characterized by acute right ventricular (RV) failure and circulatory collapse.
Case Summary:
A 32-year-old man with a mediastinal mass and malignant pericardial effusion developed severe RV dysfunction and shock shortly after pericardiocentesis. Despite escalating therapy, emergent placement of an Impella RP heart pump stabilized hemodynamics and resulted in full RV recovery within 72 hours. The patient was subsequently diagnosed with primary mediastinal large B-cell lymphoma and discharged to continue chemotherapy.
Discussion:
PDS carries significant mortality risk. Early diagnosis using echocardiographic and invasive hemodynamic markers is critical. This case demonstrates a novel use of the Impella RP device in malignancy-related PDS, highlighting the role of mechanical circulatory support.
Take-Home Message:
Early recognition of PDS and timely mechanical support are essential to improving outcomes in malignancy-associated tamponade.
