Summary

Distraction enterogenesis using the intestinal expansion sleeve (IES) device significantly elongated rat intestines by 30.2%. This novel technique shows promise for treating short bowel syndrome (SBS) by increasing intestinal length in vivo.

Background:

  • Short bowel syndrome (SBS) leads to malnutrition due to insufficient intestinal length.
  • Current surgical interventions for SBS have limitations and significant risks.
  • Distraction enterogenesis is a proposed method to induce intestinal lengthening.

Purpose of the Study:

  • To evaluate the efficacy of the intestinal expansion sleeve (IES) device in achieving intestinal lengthening through distraction enterogenesis in vivo.
  • To assess the histological changes and tissue remodeling in response to IES deployment.

Main Methods:

  • A Roux-en-Y jejunal segment was created in seven rats for isolated IES deployment.
  • The IES device was inserted into the isolated segment and deployed for 4 weeks.
  • Intestinal length was measured pre- and post-deployment, and histological analysis was performed.

Main Results:

  • The IES device achieved a significant 30.2% elongation of the jejunal roux limb (from 43.6 mm to 56.4 mm, p=0.043).
  • Histological analysis revealed changes in mucosal and submucosal integrity and bowel wall thickness.
  • Preservation of basal/regenerative mucosal layers was observed even with partial mucosal erosion.

Conclusions:

  • Distraction enterogenesis with the IES device successfully induced significant intestinal lengthening in vivo.
  • Histological findings suggest that functional bowel layers are maintained during distraction enterogenesis.
  • The IES device holds potential as a complementary treatment for short bowel syndrome, with future iterations possibly incorporating immunomodulators.
