Summary

Intramural pregnancy (IMP), a rare ectopic pregnancy, can be managed non-surgically. This case study shows successful conservative treatment preserving fertility.

Background:

Intramural pregnancy (IMP) is a rare ectopic pregnancy implantation within the uterine myometrium.
IMP poses significant risks including hemorrhage and uterine rupture.
Early diagnosis via ultrasound is crucial for management decisions.

Observation:

A 35-year-old patient with a history of in vitro fertilization (IVF) presented with diagnosed intramural pregnancy.
The IMP was located in the back-isthmian region of the uterus.
The patient was clinically stable and desired future fertility.

Findings:

A conservative treatment approach was employed, including mifepristone and ultrasound-guided intracavitary methotrexate.
Uterine artery embolization was deferred due to arterial anastomoses.
Successful, progressive reabsorption of the gestational sac occurred over 8 months.

Implications:

Non-surgical management is a viable option for select intramural pregnancy cases.
Conservative treatment can preserve future fertility in patients with IMP.
This approach highlights the importance of individualized treatment for rare ectopic pregnancies.
