    Background:

    • Patellectomy, the surgical removal of the kneecap, is a procedure performed for various knee conditions.

    • Understanding the late effects of patellectomy is crucial for patient counseling and surgical decision-making.

    Purpose of the Study:

    • To evaluate the long-term functional outcomes and radiographic changes following patellectomy.
    • To assess the impact of patellectomy on occupational status and patient satisfaction.
    • To determine the incidence of femoro-tibial arthrosis after patellectomy.

    Main Methods:

    • A retrospective review of 28 patellectomies in 27 patients was conducted.
    • Patients were re-examined at an average follow-up of 20 years.
    • Functional outcomes were assessed using a rating scale (excellent, good, fair), and quadriceps strength was measured. Radiographs were evaluated for arthrosis.

    Main Results:

    • Overall, 12 knees were rated excellent, 10 good, and 6 fair at long-term follow-up.
    • Despite strenuous work in 12 patients, only 3 changed occupations due to their surgery.
    • Quadriceps muscle strength averaged two-thirds of the contralateral limb, and only 4 patients showed signs of femoro-tibial arthrosis.

    Conclusions:

    • Patellectomy can lead to satisfactory long-term functional outcomes with a low rate of significant complications.
    • The procedure appears to have a minimal negative impact on occupational demands and progression to knee arthrosis.
    • Further research should explore patient-reported outcome measures and specific rehabilitation protocols to optimize recovery after patellectomy.
