The manifestation of aqueous humor relapse by acute lymphoblastic leukemia (ALL) on 18 F-FDG PET/CT is rarely reported. Herein, we report a rare representation of bilateral anterior chamber involvement by relapsed ALL with hypermetabolic activity observed on FDG PET/CT. This case emphasizes the higher sensitivity of 18 F-FDG PET/CT compared with CT/MRI in detecting aqueous humor relapse in patients with ALL, highlighting its potential as a noninvasive diagnostic tool.
