The vestibular system is a set of inner ear structures that provide a sense of balance and spatial orientation. This system is comprised of structures within the labyrinth of the inner ear, including the cochlea and two otolith organs—the utricle and saccule. The labyrinth also contains three semicircular canals—superior, posterior, and horizontal—that are oriented on different planes.

Background:

  • Current methods for evaluating asymptomatic inner ear dysfunction are inadequate.
  • 100-Hz sound stimulation has shown efficacy in treating vestibular dysfunction in Meniere's disease.

Observation:

  • Two female patients with endolymphatic hydrops (EH) but no hearing loss or vestibular symptoms were evaluated.
  • One patient had hearing difficulty, the other otalgia.

Findings:

  • Cervical vestibular-evoked myogenic potential (cVEMP) amplitudes did not change in ears without EH after 100-Hz sound stimulation.
  • cVEMP amplitudes significantly increased in ears with vestibular EH post-stimulation, indicating potential recovery.

Implications:

  • 100-Hz sound stimulation may serve as a novel diagnostic tool for asymptomatic vestibular dysfunction in EH.
  • This method could guide early medical intervention by assessing recovery potential.
