    Background:

    • Lupus-associated thrombocytopenia is a serious autoimmune condition.
    • High-dose prednisone is a common treatment but can have significant side effects or be contraindicated.
    • Alternative therapies are needed for refractory or contraindicated cases.

    Observation:

    • Danazol was administered to three patients with lupus-associated thrombocytopenia.
    • Two patients had previously failed high-dose prednisone, and one had contraindications.
    • Platelet counts increased in all patients receiving danazol.

    Findings:

    • Danazol therapy led to increased platelet counts in all three patients.
    • Two out of three patients developed a maculopapular rash.
    • The rash resolved upon discontinuation of danazol and recurred upon re-challenge in one patient.

    Implications:

    • Danazol shows promise as a treatment for lupus-associated thrombocytopenia.
    • Careful patient monitoring for rash development is crucial during danazol therapy.
    • Further clinical evaluation of danazol for this indication is warranted.
