[A case of brucella endocarditis in Tunisia]

Brucella endocarditis, a rare complication of brucellosis, is difficult to diagnose. Early diagnosis and a multidisciplinary approach including surgery and antibiotics improve outcomes for this serious infection.

Observation:
- A 59-year-old man with a mechanical aortic prosthesis presented with heart failure, prosthetic disconnection, and aortic annulus abscess.
- Negative blood cultures initially complicated diagnosis, but serologic testing and surgical specimen culture confirmed Brucella melitensis infection.
Findings:
- The patient underwent prosthesis replacement, abscess drainage, and prolonged antibiotic therapy (doxycycline, cotrimoxazole, rifampicin).
- Despite postoperative complications, the patient achieved favorable hemodynamic and infectious outcomes.
Implications:
- This case underscores the necessity of a multidisciplinary approach for diagnosing BE.
- Early diagnosis via targeted serologic testing and cardiac imaging, coupled with surgical intervention and appropriate antibiotics, is crucial for improving patient prognosis.
