We report the trans-brachial buddy balloon technique in a challenging case undergoing transfemoral transcatheter aortic valve implantation. Our case highlights the use of the low-profile Trival balloon to facilitate trans-brachial balloon assistance when the contralateral femoral artery is unavailable due to mechanical cardiac support.
