Summary

Tumor budding (TB) is a significant predictor of lymph node metastasis (LNM) and local recurrence in early-stage colorectal cancer (CRC). This finding aids in selecting appropriate surgical treatments for pT1 CRC patients.

Background:

  • Population screening increases early colorectal cancer (CRC) detection (pT1 stage).
  • Endoscopic resection is standard for pT1 CRC, but lymph node metastasis (LNM) and recurrence risk necessitate further treatment.
  • Tumor budding (TB) is a known adverse outcome biomarker in pT1 CRC.

Purpose of the Study:

  • To investigate the association between TB and LNM in pT1 CRC.
  • To clarify the relationship between TB and local recurrence in pT1 CRC.
  • To provide evidence for refining surgical treatment selection strategies.

Main Methods:

  • Systematic review of MEDLINE and EMBASE databases.
  • Meta-analysis of 57 observational studies (24,956 patients) using a random-effects model.
  • Evaluation of TB's association with LNM and local recurrence.

  • High-grade TB strongly correlates with LNM (RR=4.04) and local recurrence (RR=2.35).
