Abstract:
The recent study by Nicholas Li et al., "Molecular features of T and N stage progression in laryngeal cancer," presents a thorough examination of genomic, epigenetic, and transcriptional alterations associated with tumor and nodal progression in HPV-negative laryngeal squamous cell carcinoma (LSCC). This commentary discusses the clinical implications of several important findings, including NOTCH1 mutations being enriched in late-stage supraglottic tumors and poor overall survival, as well as stage-specific DNA methylation events, such as HOXB7/HOXB9 and JUN, that could also serve as markers of tumor aggressiveness. These findings raise questions about whether conventional anatomic stage suffices, and these findings also support a paradigm shift to account for differences in molecular features in risk stratification. The commentary encourages validation in future prospective cohorts and functional studies to clarify the mechanistic roles of these alterations. Incorporating such biomarkers into clinical decision-making for patients with LSCC could lead to more precise staging, personalized surveillance strategies, and biomarker-driven therapies as appropriate. The original study provides very important foundational work for the progression of precision oncology in head and neck cancer.
