Background:

  • Nail involvement in pemphigus (NIP) is often underestimated and can indicate disease severity.
  • Diagnosing NIP is challenging due to its potential to mimic other nail conditions.

Purpose of the Study:

  • To provide an overview of NIP.
  • To highlight the role of dermoscopy in identifying NIP.

Main Methods:

  • A monocentric prospective study conducted from January 2018 to December 2022.
  • Clinical and dermoscopic evaluation of patients with NIP.

Main Results:

  • Six out of 32 patients had NIP, predominantly pemphigus vulgaris (83%).
  • Most NIP cases (83%) were severe, with frequent exacerbations.
  • Onycholysis and subungual hemorrhage were the most common dermoscopic findings (83%).

Conclusions:

  • Dermoscopic features including subungual hemorrhage, onycholysis, and paronychia are crucial for differentiating NIP from other nail conditions.
