This case study details a 79-year-old man diagnosed with Epstein-Barr virus (EBV)-positive diffuse large B-cell lymphoma (DLBCL) and a synchronous T-cell lymphoma. Both were successfully treated with R-CHOP chemotherapy.

Background:

  • Epstein-Barr virus (EBV)-positive diffuse large B-cell lymphoma (DLBCL) is an aggressive malignancy often linked to immunosenescence in older adults.
  • Primary cutaneous lymphomas represent a diverse group of non-Hodgkin lymphomas primarily affecting the skin.

Observation:

  • A 79-year-old male presented with violaceous skin nodules, diagnosed as primary cutaneous EBV-positive DLBCL.
  • Diagnostic workup revealed a concurrent peripheral T-cell lymphoma, not otherwise specified (NOS), without systemic B-cell involvement.

Findings:

  • Histopathological and immunohistochemical analyses confirmed the diagnoses.
  • The patient achieved a complete response to R-CHOP chemotherapy for both lymphomas.
  • Sustained remission was observed one year post-treatment.

Implications:

  • Age is a significant risk factor for EBV-related malignancies.
  • Epstein-Barr encoding region (EBER) and CD30 testing are crucial for accurate diagnosis.
