This case report details a rare left-sided broncho-biliary fistula (BBF) successfully treated with laparotomy alone. This approach avoided thoracotomy, demonstrating effective management for selected hydatid BBF cases.
Area of Science:
- Hepatobiliary Surgery
- Thoracic Surgery
- Parasitic Infections
Background:
- Broncho-biliary fistula (BBF) is a rare complication connecting the biliary tract and bronchial tree.
- Left-sided BBF of hydatid origin is exceptionally uncommon and poorly documented.
Observation:
- A 34-year-old woman presented with a left-sided BBF due to a hepatic hydatid cyst, exhibiting bilioptysis and refractory pneumonia.
- Imaging revealed a type IB BBF, with laboratory tests showing significant hepatic cytolysis and cholestasis.
Findings:
- Surgical management via laparotomy included cystectomy, fistula closure, and diaphragmatic repair.
- The patient experienced no intraoperative complications, with successful treatment achieved through an abdominal approach.
Implications:
- This case highlights the efficacy of laparotomy alone for selected left-sided hydatid BBF, potentially avoiding thoracotomy.
- Precise preoperative imaging is crucial for determining the optimal surgical strategy, emphasizing minimally invasive options.
