Are defaulters with paucibacillary leprosy a problem?

K Jesudasan, D Bradley, M Christian

    Summary

Patients with paucibacillary leprosy who stopped dapsone monotherapy early did not show a high risk of relapse. This suggests that treatment defaulters are not a significant issue for leprosy control programs.

    Area of Science:

    • Medical Research
    • Infectious Diseases
    • Public Health

    Background:

    • Leprosy treatment requires adherence to prescribed medication courses.
    • Dapsone monotherapy was historically used for paucibacillary leprosy.
    • Patient default from treatment can impact disease control efforts.

    Purpose of the Study:

    • To assess the risk of relapse in paucibacillary leprosy patients who defaulted from dapsone monotherapy.
    • To determine if defaulting patients pose a significant challenge to leprosy control.

    Main Methods:

    • Retrospective analysis of 421 paucibacillary leprosy patients.
    • Calculation of relapse rates based on person-years of risk.
    • Evaluation of relapse occurrences after varying periods of interrupted dapsone monotherapy.

    Main Results:

    • A total of 2,162 person-years of risk were observed.
    • Ten patients relapsed during the study period.
    • The crude relapse rate was 2.4%, equating to 4.6 per 1000 person-years.

    Conclusions:

    • Defaulters from paucibacillary leprosy treatment with dapsone monotherapy do not present a substantial problem for leprosy control.
    • The observed low relapse rate suggests that interrupted treatment in this cohort did not lead to widespread treatment failures.
    • Further research may explore optimal strategies for managing defaulters in leprosy programs.
