[Effectiveness of rivaroxaban (Riqulatron) for prevention of venous thromboembolic complications in total knee

Objective:
To evaluate clinical and pharmacoeconomic effectiveness of rivaroxaban (Riqulatron) as a preventive measure for venous thromboembolic complications after total knee replacement compared to multimodal thromboprophylaxis (the so-called «bridge» scheme).
Material And Methods:
We analyzed prevention of venous thromboembolic complications. Patients with advanced coxarthrosis and total knee replacement were divided into two groups. In the control group (n=32), postoperative multimodal scheme («bridge») was used as a preventive measure for venous thromboembolic complications (calcium nadroparin 38 IU/kg subcutaneously daily throughout 3 days after surgery followed by oral rivaroxaban (Riqulatron) 10 mg daily throughout 35 days after surgery). In the main group (n=27), patients took Riqulatron 10 mg daily from the first postoperative day throughout 5 weeks.
Results:
There was similar clinical efficacy of prevention in both groups. In both groups, we observed 1 case of clinically insignificant deep vein thrombosis not registered before surgical intervention. At the same time, clear economic effect was noted in the main group. When extrapolated to surgical activity of federal center, this effect will amount to at least 2 million rubles.
Conclusion:
Despite similar clinical efficacy, Riqulatron demonstrated more obvious pharmacoeconomic effect compared to multimodal scheme.
