The Role of Frontal Sinus Size and Crista Galli Aeration in Frontal Sinusitis

Background:
Recently, researchers have conducted various studies investigating the relationship between frontal sinus size and frontal sinusitis.
Purpose:
To determine the relationship between frontal sinus size and crista galli aeration with frontal sinusitis.
Methods:
A retrospective evaluation was conducted on 1.206 frontal sinus images obtained from 603 patients aged over 20 who underwent paranasal computed tomography at the authors' institution between October 2023 and June 2024. The frontal sinus sizes were classified into 4 groups (aplasia, hypoplasia, medium, and hyperplasia) based on the Guerram classification in the coronal plane, made by consensus decision of 2 experienced radiologists.
Results:
Based on right frontal sinus size, the most common type was the medium type, observed in 58.54% (n = 353) of cases. Similarly, the left frontal sinus was predominantly of the medium type, accounting for 52.74% (n = 318) of the cases. Crista galli aeration was detected in 9.62% (n = 58) of patients. Regarding the right Keros classifications, type 2 was the most frequently identified, occurring in 54.73% (n = 330) of cases. Similarly, for left Keros classifications, type 2 was the most prevalent, observed in 50.08% (n = 302) of cases. No statistically significant association was found between right frontal sinus size classifications and frontal sinusitis ( P = 0.693). Likewise, no statistically significant relationship was found between left frontal sinus size classifications and frontal sinusitis ( P = 0.317).
Conclusion:
No association was observed between frontal sinus size and crista galli aeration with frontal sinusitis. However, a correlation was identified between the enlargement of the right olfactory fossa and frontal sinusitis.
