Customized temporomandibular joint (TMJ) replacement surgery significantly reduces pain and improves mouth opening. This TMJ treatment is effective for various diagnoses, offering long-term stability up to 7 years.

Area of Science:

  • Oral and Maxillofacial Surgery
  • Biomaterials Science
  • Orthopedic Surgery

Background:

  • Temporomandibular joint (TMJ) disorders encompass degenerative diseases, ankylosis, and craniofacial syndromes.
  • Previous surgeries and degenerative joint disease are associated with increased pain and complexity in TMJ cases.

Purpose of the Study:

  • To identify variables associated with the success of TMJ replacement interventions.
  • To evaluate the efficacy and long-term stability of customized alloplastic TMJ systems.

Main Methods:

  • Retrospective study of 31 patients (47 prostheses) undergoing TMJ replacement between 2018-2023.
  • Statistical analysis using Student's t-test (p < 0.05) to compare pre- and postoperative outcomes.
  • Follow-up duration ranged from 1 to 7 years.

Main Results:

  • Significant reduction in pain scores (p < 0.0001) from 5.5 to 2.2.
  • Significant increase in interincisal opening (p < 0.0001) from 25.85 mm to 35.93 mm.
  • 74.4% of patients required complementary facial osteotomies.

Conclusions:

  • Customized TMJ replacement is an efficient and effective treatment for diverse TMJ pathologies.
  • The intervention demonstrates stability up to 7 years post-surgery.
  • Treatment benefits patients not yet in end-stage TMJ disease.
