Abstract:
Non-tuberculous mycobacteria (NTM) have emerged as one of the most significant opportunistic pathogens, presenting challenges in diagnosis and treatment due to their diverse species, intrinsic drug resistance, and rising global incidence. Advanced molecular techniques, including next-generation sequencing (NGS), polymerase chain reaction (PCR), and matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS), have significantly improved the accuracy and speed of species identification, enabling more precise and timely diagnosis. Phylogenetic studies utilizing whole-genome sequencing (WGS) have deepened our understanding of NTM evolution, genetic diversity, and pathogenicity, refining species classification and epidemiological tracking. Despite these advancements, key challenges remain, including the lack of standardized diagnostic criteria, prolonged treatment durations, and emerging antimicrobial resistance. Integrating multi-omic technologies, artificial intelligence-driven diagnostics, and innovative therapeutic strategies holds great promise for overcoming these barriers. Future research must focus on translating molecular discoveries into clinical applications to enhance NTM diagnosis, treatment, and surveillance. By leveraging these advancements, the medical community can improve patient outcomes, reduce the global burden of NTM infections, and advance precision medicine in the fight against these challenging pathogens. This review highlights recent molecular diagnostics, phylogenetics, and clinical management breakthroughs transforming NTM research and patient care.
