Summary

The Response Assessment in Pediatric Neuro-Oncology (RAPNO) imaging criteria for diffuse intrinsic pontine glioma (DIPG) showed high concordance between radiologists. This supports its use in clinical trials for pediatric neuro-oncology patients.

Background:

  • The Response Assessment in Pediatric Neuro-Oncology (RAPNO) working group established criteria for diffuse intrinsic pontine glioma (DIPG).
  • The International DIPG/DMG Registry (IDIPGR) provided a patient sample for evaluating these criteria.

Purpose of the Study:

  • To assess the operational performance of the RAPNO DIPG imaging criteria.
  • To evaluate inter-reader agreement for response assessment in DIPG using RAPNO criteria.

Main Methods:

  • Retrospective analysis of longitudinal MRIs from 46 DIPG patients.
  • Independent review of MRIs by two pediatric neuro-radiologists using RAPNO DIPG imaging criteria.
  • Categorization of tumor response (CR, PR, SD, PD) and assessment of inter-reader concordance.

Main Results:

  • 84% (68% concordant, 25% minor discordant) of 124 MRI comparisons showed agreement or minor discordance.
  • No complete responses (CR) were reported.
  • Discordant measurements were often near the boundaries for partial response (PR) or progressive disease (PD), with a median difference of 2 mm.

Conclusions:

  • The RAPNO DIPG imaging criteria demonstrated high applicability and concordance (93%) between readers.
  • Minor discordances were frequently observed at the thresholds of response categories.
  • These findings support the utility of RAPNO DIPG criteria in clinical practice and research.
