Rifampicin-induced renal failure.

J R Cohn, D L Fye, J M Sills

    Tubercle
    |December 1, 1985
    PubMed
    Summary

Rifampicin can rarely cause kidney failure in tuberculosis patients, sometimes early in treatment. Intermittent rifampicin therapy significantly increases the risk of developing this serious renal complication.

    Area of Science:

    • Nephrology
    • Infectious Diseases
    • Pharmacology

    Background:

    • Rifampicin is a critical antibiotic for tuberculosis treatment.
    • Renal failure is a known, albeit rare, adverse effect of rifampicin therapy.
    • Understanding the risk factors for rifampicin-induced renal complications is crucial for patient safety.

    Observation:

    • Two cases of acute renal insufficiency attributed to rifampicin are presented.
    • The first case involved oligo-anuric renal failure occurring after only 9 doses of rifampicin.
    • The second case developed renal failure during continuous inpatient rifampicin therapy.

    Findings:

    • A literature review identified 83 additional cases of rifampicin-induced renal insufficiency.
    • Early onset of renal failure, even after limited exposure, is possible.
    • Intermittent or interrupted rifampicin administration is a significant risk factor.

    Implications:

    • Clinicians should be vigilant for early signs of renal dysfunction in patients treated with rifampicin.
    • Intermittent dosing strategies for rifampicin may warrant re-evaluation due to increased renal risk.
    • Further research into the mechanisms of rifampicin nephrotoxicity is needed to optimize tuberculosis treatment regimens.
