Traumatic Brain Injury l: Introduction

DefinitionTraumatic brain injury, or TBI, is a disturbance of normal brain function induced by an external mechanical force, such as a direct blow to the head or a penetrating injury. It can affect both brain structure and function, producing a wide range of clinical outcomes. TBI is a heterogeneous condition, meaning its effects may differ based on the type, location, and severity of the injury.Basis of ClassificationTBI is classified based on severity, injury mechanism, or pathophysiology. Summary

A fall revealed a giant-cell tumour of bone (GCTB) in a young man

Area of Science:

  • Orthopedic Surgery
  • Oncology
  • Radiology

Background:

  • Giant-cell tumour of bone (GCTB) is a primary bone tumour typically occurring in skeletally mature individuals.
  • GCTB commonly affects the epiphysis of long bones, particularly around the knee.

Observation:

  • A 25-year-old male presented with acute left knee pain and swelling after a fall, with a history of several months of left knee pain.
  • Physical examination revealed a swollen left knee with limited range of motion.
  • Radiographs showed a lytic lesion on the left lateral femoral condyle.

Findings:

  • Computed tomography (CT) and magnetic resonance imaging (MRI) were performed.
  • Open biopsy and immunohistochemical staining confirmed the diagnosis of GCTB, with tumour cells positive for H3G34W.

  • The patient underwent surgical treatment including curettage, bone grafting, and internal fixation of the left distal femur.
  • Implications:

    • This case underscores that trauma can be the initial presentation of an undiagnosed bone tumour.
    • Clinicians should maintain a high index of suspicion for bone tumours in patients presenting with persistent or acute joint pain, especially after injury.
    • Early diagnosis and appropriate management of GCTB are crucial for optimal patient outcomes.
