Bone Remodeling

Bone Remodeling

Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.

Summary

Primary renal osteosarcoma, a rare and aggressive cancer, is diagnosed through histopathology and immunohistochemistry. Early detection via routine imaging is crucial for managing this challenging malignancy.

Background:

Primary renal osteosarcoma is an exceptionally rare and aggressive malignancy.
It presents with nonspecific features, necessitating histopathological and immunohistochemical confirmation.

Observation:

A 56-year-old man was incidentally diagnosed with a calcified left renal mass during routine screening.
Contrast-enhanced CT revealed a heterogeneous lesion at the renal hilum.
Laparoscopic radical nephrectomy was performed, with histopathology confirming osteoblastic osteosarcoma.

Findings:

The tumor exhibited osteoid and chondroid matrix, with positive Vimentin and SATB2 expression.
Differential diagnoses, including sarcomatoid carcinoma and retroperitoneal liposarcoma, were excluded.
The patient developed bone metastases six months post-surgery.

Implications:

Surgical resection is the primary treatment for renal osteosarcoma.
The role of adjuvant therapy requires further investigation due to limited data.
This case highlights the significance of routine imaging in identifying rare renal malignancies early.
