Left ventricular diverticulum is a very rare congenital cardiac malformation. Due to variations in the size, location, and morphology of the diverticulum, the clinical symptoms of patients differ significantly. Herein, we present a case of a 44-year-old male patient in whom a sub-mitral valve muscular diverticulum of the left ventricle was incidentally discovered during cardiac imaging performed for other conditions. The unique feature is that this diverticulum protruded into the ventricular chamber. This particular type of diverticulum has not been previously reported.
