    Background:

    • Autonomic dysfunction is common in diabetes.
    • Sleep-related blood pressure regulation can be impaired in diabetic patients.

    Purpose of the Study:

    • To investigate nocturnal blood pressure patterns in Type I and Type II diabetes during sleep.
    • To compare sleep blood pressure regulation between diabetic subtypes and healthy controls.

    Main Methods:

    • Serial blood pressure monitoring during sleep using an automated Doppler system.
    • Inclusion of 7 normal controls, 11 Type I (insulin-dependent), and 6 Type II (non-insulin-dependent) diabetic patients.
    • Bedside autonomic function tests were performed on all subjects.

    Main Results:

    • Type I diabetic patients displayed abnormal systolic blood pressure throughout the night.
    • Unlike controls and Type II diabetics, Type I patients lacked a progressive rise in blood pressure during early morning hours.
    • Blood pressure fluctuations during REM sleep were significantly different in Type I diabetics compared to non-REM sleep and other groups.

    Conclusions:

    • Type I diabetes is associated with distinct abnormalities in nocturnal blood pressure regulation during sleep.
    • Sleep-specific blood pressure patterns may serve as indicators of autonomic dysfunction in diabetes.
    • Further research is warranted to understand the clinical implications of these findings.
