    Background:

    • Transchondral talar dome fractures are a significant cause of persistent ankle pain and delayed return to athletic activity.
    • Late diagnosis and neglected treatment of these osteochondral lesions can lead to chronic disability in athletes.

    Purpose of the Study:

    • To present clinical findings, diagnostic aids, and treatment methods for transchondral talar dome fractures.
    • To evaluate the outcomes of conservative and surgical interventions for these injuries.

    Main Methods:

    • Retrospective review of 39 patients (41 fractures) treated between 1976 and 1983.
    • Average follow-up duration of 5.1 years.
    • Analysis of conservative (n=22) and surgical (n=17) treatment outcomes.

    Main Results:

    • Conservative treatment yielded good results in 12/22 patients and moderate in 4/22.
    • Surgical treatment resulted in good outcomes for 9/17 patients and moderate for 3/17.
    • Four surgically treated patients experienced poor results, impacting sports or work activities.

    Conclusions:

    • Transchondral talar dome fractures require timely diagnosis and management.
    • Both conservative and surgical approaches can yield favorable outcomes, but surgical intervention carries a risk of poor results.
