Background:

  • Recurrent acute tonsillitis (RAT) frequently necessitates tonsillectomy (TE).
  • Laser TE is an emerging alternative to traditional cold steel dissection, but its comparative effectiveness and safety require thorough investigation.
  • This study addresses the clinical uncertainty surrounding laser TE for RAT treatment.

Purpose of the Study:

  • To systematically review and meta-analyze the efficacy and safety of laser TE versus cold steel dissection TE in patients diagnosed with RAT.
  • To compare key surgical outcomes including intraoperative blood loss, operation duration, postoperative pain, quality of life, and postoperative hemorrhage.

Main Methods:

  • A systematic literature search was conducted across PubMed, Embase, and Cochrane Central databases.
  • Nine studies, including six randomized controlled trials with 612 patients, were included in the meta-analysis.
  • Statistical analyses were performed using RStudio and Review Manager, with a prospective protocol registered in PROSPERO.

  • Laser TE demonstrated significantly lower intraoperative blood loss (MD -35.89 mL) and shorter operation times (MD -10.46 min) compared to cold steel dissection.

  • High heterogeneity was noted for blood loss (I²=100%) and operation time (I²=99%) outcomes.

Conclusions:

  • Laser TE offers advantages in reduced intraoperative blood loss and shorter procedure times for RAT patients compared to cold steel dissection.
  • The risk of hemorrhage, postoperative pain, and quality of life appear comparable between the two tonsillectomy techniques.
  • Further high-quality randomized trials are warranted to elucidate long-term outcomes, particularly concerning post-tonsillectomy sore throat, after laser TE.
