    Background:

    • Pentazocine is an opioid analgesic with a complex history of use and misuse.
    • Understanding the true incidence and characteristics of pentazocine dependence is crucial for clinical management and public health.
    • Previous reports suggest pentazocine dependence is less common than other opioids, but data remains limited.

    Observation:

    • Over a 5-year period, 60 pentazocine notifications were received, with the majority deemed non-dependent.
    • Of 46 usable notifications, only 8 cases were classified as true pentazocine dependence.
    • Most reported cases involved temporary use, non-use, chronic pain treatment, or polysubstance abuse.

    Findings:

    • True pentazocine dependence is rare, with an incidence significantly lower than initially notified.
    • Individuals classified as dependent or polysubstance abusers consumed higher average daily doses (11 ampoules) compared to those using it for pain.
    • Withdrawal symptoms and social dangers associated with pentazocine dependence appear less severe than those of traditional opiates.

    • Pentazocine dependence is a less significant public health concern compared to established opiate dependencies.
    • Clinical interventions for pentazocine misuse should differentiate between dependence, temporary use, and polysubstance abuse.
    • Oral administration of pentazocine is associated with a near-zero incidence of dependence, suggesting a safer route of administration.
