Summary

Beta-blocker therapy effectively reduces or prevents exercise-induced intraventricular pressure gradients (IVPGs) in adolescents. This treatment also significantly alleviates associated exercise-related symptoms and abnormal electrocardiogram findings.

Background:

  • Treadmill exercise stress echocardiography (ESE) is a safe and feasible diagnostic tool in pediatric patients.
  • Intraventricular pressure gradients (IVPGs) are frequently observed during exercise in adolescents, absent at rest, and offer insights into exercise-related symptoms and ECG abnormalities.
  • Regional wall motion abnormalities (RWMAs) have limited diagnostic utility in this population.

Purpose of the Study:

  • To assess the impact of beta-blocker therapy on the development of IVPGs in adolescents with concerning symptoms or ECG findings.
  • To evaluate the efficacy of beta-blockers in managing exercise-induced hemodynamic changes.

Main Methods:

  • Exercise stress echocardiography (ESE) was repeated in 66 adolescents who initially presented with IVPGs.
  • Participants underwent follow-up assessment while on beta-blocker therapy, with normal resting echocardiograms.
  • Echocardiographic assessments included comprehensive two-dimensional and Doppler evaluations at baseline and during beta-blocker treatment.

Main Results:

  • Beta-blocker therapy either eliminated IVPGs in 37 adolescents or significantly reduced them in 29 others (mean gradient decreased from 105 ± 38 mmHg to 58 ± 32 mmHg).
  • Peak exercise heart rate decreased from 178 ± 15 bpm to 157 ± 9 bpm under beta-blocker treatment.
  • Exercise-induced clinical symptoms, initially present in 47 adolescents, were reduced to only seven during treatment.

Conclusions:

  • Oral beta-blocker therapy is effective in preventing or significantly reducing exertional IVPGs in adolescents with symptoms or abnormal ECGs.
  • Hemodynamic improvements achieved with beta-blockers correlate with a substantial resolution of clinical symptoms, observed in 85% of symptomatic adolescents.
  • Beta-blocker therapy represents a valuable therapeutic strategy for managing exercise-induced cardiovascular abnormalities in this pediatric cohort.
