Purpose of the Study:

  • To evaluate the inter-reader reliability and diagnostic accuracy of the MSKI-RADS among early-career radiologists.
  • To compare the diagnostic performance of MSKI-RADS with qualitative diagnoses.

Main Methods:

  • A retrospective multicenter study analyzed 210 MRIs of proven musculoskeletal infections.
  • Twenty-two early-career radiologists (residents, fellows, junior attendings) evaluated the cases after training.
  • Readers assigned MSKI-RADS scores, qualitative diagnoses, and confidence levels; inter-reader agreement (ICC) and accuracy were calculated.

Main Results:

  • Moderate inter-reader agreement (ICC: 0.57) was observed among the 22 readers.
  • Overall MSKI-RADS reader accuracy was 66%, significantly higher than qualitative diagnosis accuracy (59%).
  • True positive rates for bony infections (classes IV and V) were high, reaching 82%.

  • MSKI-RADS is an accurate and reliable MRI-based classification system for diagnosing musculoskeletal infections.
  • The system is effective for early-career radiologists, performing comparably to expert readers.
  • MSKI-RADS aids in the diagnosis and grading of extremity musculoskeletal infections.
