Cardiovascular disease (CVD) patients often experience depression, impacting care. Clinical practice guidelines (CPGs) inadequately address depression screening and treatment, highlighting a need for integrated mental health support in CVD management.
Area of Science:
- Cardiology
- Psychiatry
- Public Health
Background:
- Patients with cardiovascular disease (CVD) have higher depression rates than the general population.
- Co-occurring CVD and depression worsen quality of life, outcomes, and increase mortality.
- Depression complicates disease management, medication adherence, and lifestyle changes in CVD patients.
Purpose of the Study:
- To evaluate how cardiovascular disease clinical practice guidelines (CPGs) address depression.
- To assess the inclusion of practical management guidance for depression within CVD CPGs.
Main Methods:
- Systematic literature search of CPGs published between 2013 and 2024.
- Analysis of 65 CPGs covering various cardiovascular and cerebrovascular conditions.
- Evaluation of guideline content regarding depression screening, treatment, and professional involvement.
Main Results:
- 71% of CPGs acknowledged depression as a risk factor, but only 23% offered both screening and treatment recommendations.
- Few CPGs (12%) referred to dedicated depression CPGs; stroke CPGs provided most recommendations (68%).
- Limited guidance on drug-disease interactions (3%) and no treatment recommendations in women-specific CPGs were found.
Conclusions:
- Current CVD CPGs insufficiently address depression, lacking comprehensive screening and treatment guidance.
- Involving mental health specialists in CPG development is crucial for holistic patient care.
- Tailored depression management strategies for diverse patient populations are needed within CVD CPGs.
